Visit Your Family Doctor
If you have a family doctor, you can discuss your sleep or mental health concerns with them and ask them to send us a referral. If you’d prefer, you can also refer yourself directly using our self-referral form.
They Will Send Us A Referral
Your family doctor will prepare all the relevant information, fill out our referral pad, than send us the completed paperwork.
We Triage The Referral
Our medical director will review your referral and determine which specialist will best suit your needs.
We Contact You Immedietly
One of our medical professionals will contact you and schedule an assessment with our specialist.
Your Assessment Appointment
Your assessment appointment will be approximately one hour long. We prefer that you come and meet your physician in person.
Tailored Follow Up Care
We provide tailored follow up care that best suits your needs and schedule. Either in person, by Zoom professional or by phone calls.
